What Is a Basal Implant?
A basal implant is a type of dental implant anchored in the deeper, denser basal (cortical) bone of the jaw rather than the softer alveolar bone used by conventional implants. The concept was first proposed by Dr. Jean-Marc Julliet in 1972 and later refined by Dr. Gérard Scortecci and Dr. Stefan Ihde in 1997, giving rise to what is also called bicortical or cortico-basal implantology. Because the basal bone is denser and more resistant to resorption than the spongy alveolar bone above it, this bone layer has been used as an anchoring point for patients whose alveolar bone has thinned due to long-term tooth loss, gum disease, or denture wear.
Most basal implants use a single-piece design, where the implant body and abutment are fused rather than assembled from separate parts. This is one reason they are sometimes considered for immediate or early loading protocols, subject to case-specific evaluation.
